Police are urging people to keep a lookout for 60-year-old Lake Macquarie resident Patrick Thaiday, who wandered off from his Macquarie Hills home on Sunday.
Mr Thaiday suffers from diabetes and dementia and was last seen wearing a grey ‘Everlast’ jumper and track pants.
Police say they hold grave concerns for his welfare.
“He’s wandered off on about 8 occasions in the past 12 months but he’s usally found within 24-48 hours, but on this occasion we’re approaching three days now and that’s very unusual,” said Chief Inspector Dave Matthews.
Anyone who sees Mr Thaiday should contact police.

NSW Police are continuing to search for a missing man from Lake Macquarie.
60-year-old Patrick Thaiday was last seen at his home in Macquarie Hills on Sunday December 6.
Wollongong Police said he has been previously been found in the Wollongong area and are encouraging people to share the appeal to find him.
Police Rescue, SES and other agencies have been searching bushland in Macquarie Hills.
Police and family members hold grave concerns for his welfare due to medical conditions and that he’s never gone missing this long.
Patrick is described as being of Aboriginal/Torres Strait Islander appearance, about 175cm tall, with a medium build, short black hair, scruffy facial hair and brown eyes.
